Electronic 1095-C forms for 2022 are now available to view and print online through Employee Self Service. Paper forms were mailed on March 1, to each recipient’s permanent address on file. An employee who consents to receiving the Form 1095-C online will not receive a paper copy.
Employers are required to furnish employees Form 1095-C Employer-Provided Health Insurance Offer and Coverage document. The 1095-C form is part of the Affordable Care Act (ACA) that requires employers to provide full-time employees (averaging 30 hours or more per week) documentation of whether health insurance coverage was provided for the calendar year.
Employees are required to provide consent to view or print a 1095-C form online (current year or a prior year). If you already gave your consent to receive your 1095-C online, you do not need to give consent again. Once consent is given, it carries forward each year and does not need to be repeated.
To provide consent to receive/view any 1095-C electronically:
- Log on to my.semo.edu
- Under Tools select Banner, then select Banner PROD Self-Service
- Select Employee, next select Tax Forms, then select the Electronic W-2 Consent
- Check the Consent to Receive 1095-C Electronically box
- Click on the Submit button
The process to view and print the electronic 1095-C is as follows:
- Log on to my.semo.edu
- Under Tools select Banner, then select Banner PROD Self-Service
- Under Tax Forms, Select 1095 Employer-Provided Health Insurance Offer and Coverage Statement
- Select the appropriate year you would like to view
- Click on the Display button.
To print a copy of the form, click on the Printable 1095-C button on the bottom left side of the form.
