All performance management forms are available through the workflow system.
Employees and supervisors may access the forms at myworkflows.semo.edu.
All employees hired prior to Oct. 1, 2023, should receive an evaluation. Self-Evaluations are the first step in the Annual Review process. All staff self-evaluations were due Dec. 15, 2023. Step-by-step instructions for completing all forms may be accessed on the employee portal at https://my.semo.edu. Please type “Performance Evaluation” in the search bar at the top of the portal. Select the Job/Employment Forms & Resources page. You will find instructions under the Performance Evaluation Resources section.
Detailed below are helpful hints and reminders regarding completion of the online form.

Review the Self-Evaluations submitted by your staff.

Review the Annual Evaluation online form before entering ratings and comments.

Select your staff member’s name from the drop-down menu.

Select a rating for each Core Criteria.

Please be sure to save your form.

If you are working on the form and are unable to submit, please be sure to save the form and resume later.

“Save & Resume” allows managers to save their progress and complete at a later date.

Managers will receive a link with the saved form in their email. The email will include the staff members’ name in the subject line.

If the manager follows their saved link and returns to their entry, they can pick up where they left off and complete their submission.

Managers may also find saved forms in their task list.

Print copy of the completed form and meet with staff members by February 1st.

Manager and staff member will review the printed form and discuss goals.

Final copy of the Annual Evaluation, with goals, will routed through the workflow
system for electronic signatures.

Reviewer approves evaluations and routes to Human Resources by February 22nd.
