The following forms can be downloaded as PDF files, printed, and returned to the Registrar's Office for processing. You can bring the forms to the Registrar's Office in Academic Hall Room 058 during normal business hours. The forms can also be mailed to Registrar's Office, MS3760, Southeast Missouri State University, One University Plaza, Cape Girardeau, MO 63701. You may also scan and email the form to firstname.lastname@example.org.
Using the National Student Clearinghouse, Southeast’s Registrar’s Office will take requests to process and deliver official transcripts.
The Family Education Rights and Privacy Act of 1974(Buckley Amendment) protects the privacy of students’ educational records. Certain information is considered “Public/Directory Information” and may be released by the University without a student’s permission unless it is requested to be withheld. Directory information including the following: name, address, email address, telephone listing, date and place of birth, major field of study, participation in officially recognized activities and sports, weight and height of members of athletic teams, date of attendance, full or part time enrollment status, number of hours enrolled in a semester degrees, awards received and the most recent previous educational agency or institution attended by the student, may be disclosed without the consent of the student. Students who wish to request that public/directory information be withheld may complete the appropriate form in the Office of the Registrar.
