Ensure easy access to your 2022 W-2 and 1095-C information by electing to access your information online.
Employees are required to provide consent to view or print a W-2 or 1095-C form online. If you already gave your consent to receive your W-2 or 1095-C online, you do not need to give consent again. Once consent is given, it carries forward each year and does not need to be repeated.
To provide consent to receive/view a W-2 or 1095-C electronically:
- Log on to my.semo.edu
- Select Banner Under Tools
- Select Banner PROD Self-Service
- Select Employee
- Under Tax Forms, select the Electronic W-2 Consent option
- Check the Consent to Receive W-2 Electronically box and/or
- Check the Consent to Receive 1095-C Electronically box
- Click on the Submit button
An employee who consents to receiving the Form W-2 or Form 1095-C online will not receive a paper copy. Printed paper W-2’s and 1095-C’s will be distributed to the employee address on record no later than January 31.
If you elect to receive a paper W-2, please ensure that your permanent address on file is correct. Addresses can be viewed by:
- Log on to my.semo.edu
- Select Banner Under Tools
- Select Banner PROD Self-Service
- Select Personal Information
- Select Update Addresses and Phones
- Click on Current to update address and phone information
- Click on the Submit button
