These forms help our Campus Clinic give appropriate medical treatment and advice when a student visits. We ask that you have them completed by a physician in your place of residence before you come to the United States. You do not need to submit the forms, but please have them available for your records. Completing these forms are optional, not mandatory. If you do not find them in your acceptance packet, you may print them from our website below. Please note that the full list of immunizations is not required; however, your US doctor(s) will need to know which immunizations you have taken in order to assess illness correctly.
To help the Campus Health Clinic provide you with the best care possible, you may fill out the questionnaire below. You do not have to submit the form, but it will benefit you to have it on-file in case of emergency.
- Obtain a list with the details of all your previous immunizations from medical professionals in your home country. Use this list to complete the Health Profile form (in English) and keep it with you in case the information is needed at a later date.
- Students are required to have a tuberculosis (TB) test once they arrive to the U.S. All students will visit the Campus Clinic during the 'Check-In' event on the first day of International Student Orientation for the mandatory TB Test.
- Meningitis vaccination is required for those living in on-campus housing. You may get this vaccination before you arrive to the University and send documentation to the Office of Residence Life (housing): orl@semo.edu or bring documentation with you to show during the 'Check-In' event.
- NOTE: If you do not have documentation of having received a meningitis vaccination when you arrive to the 'Check-In' event, you will be required to get the vaccination at the Campus Clinic when you have the mandatory TB Test.
- Mumps, Measles, Rubella (MMR) vaccination is highly recommended for all students. You may get this vaccination before you arrive to the U.S. and bring documentation with you to present during the 'Check-In' event. You may opt in to receive the MMR vaccination while at the Campus Clinic for the TB Test on the first day of International Student Orientation.
Campus Health Clinic
Southeast has an on-campus health clinic that offers health treatment including physical exams, laboratory services, immunizations and allergy shots, medical procedures, prescription services, wellness exams and assessment, and treatment of illness/injuries for students of Southeast Missouri State University.
Center for Behavioral Health and Accessibility
The Center for Behavioral Health and Accessibility provides individual and group counseling services to students to assist with a variety of concerns including adjustment to campus, stress management, anxiety, depression, eating disorders, substance use, interpersonal violence, and sexual assault.
