Faculty Forms

General Education forms are available as Adobe Acrobat Reader (.pdf) files and/or Microsoft Word (.doc/docx) files. You can also pick up forms in the General Education Office in Memorial Hall Room 210.

General Education Course Approval Document (CAD)

  1. Visit the Course Approval Procedures on the Provost’s page, https://www.semo.edu/provost/curriculum/course-approval-procedures.htm. New courses must be approved by the appropriate bodies prior to submission to General Education. (Course proposers are welcome to contact General Education during development for guidance and assistance.)
  2. Visit the General Education Handbook for the current year. This is obtained online from the General Education site. Review the General Education structure and program. Understand how your proposed course will fit into the General Education structure.
  3. Obtain the Guidelines for Creating a Course Approval Document (CAD Guidelines, above).
  4. Note that the first portion of the Guidelines very closely mirrors what is contained in the “Course Syllabus Format” on the Provost’s page.
  5. There are a few items on the first page (denoted in green) that are unique to General Education courses. The remainder of Sections I – VIII are for all courses.
  6. Please read the instructions for those portions of the CAD that are unique to General Education courses, including the Appendices.
  7. The completed CAD must be submitted with a completed CAD Cover Sheet.
  8. Submission deadlines are:
    1. September 15th for courses to be scheduled the following Fall semester.
    2. February 15th for courses to be scheduled the following Spring semester.
  9. Submit your CAD (including the Cover Sheet with all necessary information) to General Education, email univstudies@semo.edu.

General Education Temporary Course Request

If your department has a course that you want to teach one semester as a General Education course, please submit an email to univstudies@semo.edu with a syllabus for the course. If they approve the temporary course, please follow these steps:

  1. Get get a temporary course approval document from the Employee SS Portal, under document share, sign the form electronically, and obtain all necessary signatures.
  2. Send the electronically signed form to the General Education office via email at univstudies@semo.edu.

The Director of General Education will sign the document, and it will be sent to the Registrar's office.
