If you have attempted to apply for graduation via the portal, and were not successful, you may use a writeable .pdf version of the application. Download (save) the blank form to your computer. Then fill in the appropriate fields and save the completed document to your computer. You must SAVE the document on your computer, and then e-mail it as an attachment to firstname.lastname@example.org. To access this application, you must have Adobe Acrobat Reader loaded on your computer. If you do not have Adobe Reader, please click the button below.
NOTE: If you are using a MAC, you must use the "Save as" option to create the .pdf. Do not use the "Export to" option. For more information on how to submit the application using a MAC, click here.
If for any reason you are still unable to save the document, you may print and complete the form. If you use this option, PLEASE BE SURE THE FORM IS PRINTED IN THE PORTRAIT ORIENTATION AND FILLS THE ENTIRE SHEET OF PAPER. Please complete all fields and print clearly. The completed form may be dropped off at the Registrar's Office (Academic Hall 057) or mailed (see address above). Please do not photograph the form or send by Cloud.
